Form for report of lost, stolen or defective digital tachograph card

This form is primarily in Norwegian, but do also have text in English.
This form is used for giving report and statements to the Norwegian Public Roads Administration in case your digital tachograph card has been lost or stolen, or if the card is defective or otherwise no longer is usable, and should be completed and delivered when applying for a replacement card.

Delivering a report of lost, stolen or defective digital tachograph card that is false or that contain incorrect informations may result in prosecution for making false statement to public authority, and may be punished with a fine or sentence of imprisonment for up to two years, cf. the Norwegian Penal Code of 2005 section 221. |
General informations about driver cards
A driver card is a digital tachograph card (also called with the acronym «DTC» in English) used by drivers driving any kind of vehicles equipped with a digital tachograph.
The driver card is personal, and shall not be used by other drivers than the driver the card is issued to (cf. item 10, litra c).
A driver card is valid for five years, counting from four days after the application has been registered. Upon special request, the driver card can be issued with a shortened period of validity.
General terms for driver cards
General terms for driver cards:
«Permanent residence» means the place where you usually reside, that is for at least 185 days in each calendar year, because of personal and occupational ties. This is usually your address as registered by the national registry office («folkeregisteret») in Norway.
When applying for a driver card, you may have to provide proof of permanent residence, cf. item 3.1, litra d.
In case you already hold a valid driver card, this driver card must be handed in when applying for a new driver card, cf. item 3.1, litra b.

Application for driver card and delivery of driver card

The application form, downloaded from part 1, item 1, should be printed out, completed, and then delivered by personal attendance at the Norwegian Public Roads Administration. Delivery The driver card is sent by regular mail to your permanent residence, where it may take up to one week from delivering the application until you receive the driver card. In case you didn't hand in your current driver card when delivering the driver card application (cf. item 3.1, litra b), your must call for your new driver card at the Norwegian Public Roads Administration. |

Complaints If you, after receiving your new driver card, can substantiate that the card have incorrect informations or is defective or otherwise not usable, and this is caused by the Norwegian Public Roads Administration or the card maker, a replacement card will be issued free of charge (cf. item 3.1, litra c). |
Initial driver card application
Initial driver card application should be done:
An initial driver card application require that you have a valid driving licence and a permanent residence i Norway, cf. item 2, #1.
Driver card renewal
You may apply for a driver card renewal:
Application for renewal must be delivered not less than 15 days prior to the expiry date (cf. item 10, litra d).
If the elapsed time since the expiry date has exceeded two years, you must make a new initial driver card application, cf. item 4, litra b.
Replacement card
You may apply for a replacement card:
A lost or stolen card should also be reported to the Norwegian Public Roads Administration using a determined report form, cf. item 9.
When applying for a replacement card, you must:
The statement should be written on the determined report form downloaded from part 1, item 2. The report form should be printed out, completed, and then delivered by personal attendance to the Norwegian Public Roads Administration.
Driver card replacement due to name change and similar
You may apply for a new driver card with corrected informations if you change name or similar, provided the driver card still is valid.
If there are less than 6 months prior to the expiry date, you may alternatively apply for a driver card renewal, cf. item 5, litra a.
Exchange of foreign driver card
If you hold a foreign driver card that shall be exchanged with a norwegian driver card, this shall be done as for initial driver card application, cf. item 4, litra c.
Lost or stolen driver card
In case the driver card is lost or stolen, this should immediately be reported to the Norwegian Public Roads Administration using a determined report form downloaded from part 1, item 2. The report form should be printed out, completed, and then delivered by personal attendance at the Norwegian Public Roads Administration.
If you hold a driver card issued in Norway, and now resides in another country, you must also notify the authorities in the country where you now reside.
If the driver card were stolen while being abroad, this should also be reported to the police in the country where the card were stolen.
To obtain a new driver card to replace the lost or stolen card, you should apply for a replacement card, cf. item 6, litra b.
In case a lost or stolen driver card turn up and in your possession after you have received a replacement card, the formerly lost or stolen card should immediately be handed in to the Norwegian Public Roads Administration, or sent by regular mail using the following address:

General informations about workshop cards
A workshop card (a.k.a. fitter card) is a digital tachograph card used by fitters working with digital tachographs at an approved tachograph workshop.
The tachograph workshop is registered as the card holder, while the fitter is registered as the card user. A workshop card shall not be used by any other fitters than the fitter registered as the user of the card (cf. item 10, litra e).
A workshop card have a personal PIN code that is unique for each card, and the PIN code shall not be known by any other fitters than the fitter registered as the user of the card (cf. item 10, litra f). In case the PIN code is entered in error too many times, the workshop card will be permanently blocked, and a new workshop card with a new PIN code must be issued (cf. item 8).
A workshop card is valid for one year, counting from four days after the initial application has been registered. When the card is renewed, or in any other case where a new workshop card is issued, the period of validity is counting from the expiry date of the previous workshop card.

In case you already hold a valid workshop card for a tachograph workshop, this workshop card must be handed in when applying for a new workshop card, cf. item 3.1, litra b.
This is because it's the workshop that is the card owner, and because the workshop card shall be kept at the workshop when not in use (cf. item 10, litra g).

Delivery The workshop card is sent by regular mail to the workshop, where it may take up to one week from delivering the application until the workshop receive the workshop card. In case you didn't hand in your workshop card when delivering the workshop card application (cf. item 3.1, litra b), your must call for your new workshop card at the driver and vehicle licensing office («trafikkstasjon»). For security reasons, the PIN code is not sent with the workshop card, but is sent separately a few days after sending the workshop card. This shipment is also sent to the workshop, but will have your name on the envelope. 3.3.

Workshop card that have been blocked after incorrect PIN entries When entering the workshop card PIN code, you have four attempts to enter the correct code. At the fifth attempt, the workshop card is permanently blocked, and a new workshop card with a new PIN code must be issued. To obtain a new workshop card and a new PIN code you should apply for a replacement card, cf. item 6, litra b. Lost or stolen workshop card In case the workshop card is lost or stolen, this should immediately be reported to the Norwegian Public Roads Administration using a determined report form downloaded from part 1, item 2. The report form should be printed out, completed, and then delivered by personal attendance at the Norwegian Public Roads Administration. In case your period of employment at the workshop come to an end before the workshop card has expired, you or the workshop manager SHALL deliver the card to the Norwegian Public Roads Administration. In such cases, the card should be delivered without the PIN code to ensure that no one else can use the card.
